A Collaborative PCMH Strategy for Better Care & Outcomes

SHP, CCNC  and ActiveHealth are working together to find ways to increase member engagement for managing their personal health and health care needs.  The approach involves introducing a patient-centered medical home care delivery (PCMH) model as an enhancement to the traditional telephonic care management service that has been used up to this point. This is a first step in an evolving strategy.
